From 2009, health
care professionals will be required to use the Palliative
Care Supplementary Information Tool. This referral tool is
part of the SCTT tool suite. Further information about the SCTT

All health care professionals will make or receive referrals
on behalf of clients at some time. Therefore it is important that all
health
care professionals are familiar with the Referral Standards and their
service practice which governs how referrals between services should
occur.
